INTERNSHIP ASSESSMENT

UNIT DUTY : 

( 14/4/23 to 28/4/23  and 28/5/23 to 12/6/23)

Attended four OPD days and took complaints and history for many pateints and made PaJR groups for admitted cases .

PSYCHIATRY DUTY: ( 29/4/23 to 13/5/23)

Learned about importance of history taking and how the disease affected this daily routine and interpersonal interaction 

Saw cases in OPD  and learned how to take basic history 

Examples like :

1. Depression 

2. Anxiety disorders

3.ALCOHOL DEPENDENCE SYNDROME 

4. TOBBACO DEPENDENCE SYNDROME 

5. Scrizoprenia 

Went to  Deaddiction centre for rounds with pgs and saw the process of  councelling and could appreciate the sequence of improvement in the pateints.

PERIPHERALS : ( 14-5-23 to  28-5-2023)

WARD DUTY:

Took case history's for newly admitted cases and wrote casesheet.

Checked vitals of the pt in ward and asked about their subjective feeling of betterment from the day of admission to day of discharge

Helped my co interns by updating  SOAP notes.

Monitored patients in the ward as guided by concerned unit PG and helped them by updating the ward patients. 

Updating about the newly admitted cases in ICU,AMC and as well as WARD.

Attended rounds in the Morning for ward , icu and amc .

NEPHRO DUTY:

Monitored the pt undergoing dialysis both daycare and admitted cases

Saw the process of central line insertion.

Handled cases which came in emergencies with help of my PGs. 

Learnt about dialysis machine and what all infections a pt can get by not maintaining proper hygiene of the central line 

Learnt about drugs used in hypo and hypertension.

Seen 3 cases of young onset CKD requiring dialysis.

Typed many discharges . 

 

ICU & AMC DUTY:

Monitored vitals of all pts hourly or as per the need accordingly to condition of the pateint

Learnt about the ventilation and how to make adjustment in the machine.

Taken ABGs and have taken samples for lab investigations.

Learnt how to manage in critical situations.

Learnt stabilisation of pt in critical situations.

Seen bed sores and management( dressings) done for bedsores.

Learnt suctioning of ET tube 

Attended rounds for ICU and AMC cases.

ADDITIONALS:

Have assisted in 2 intubations.

Have put 3 ryles tube and 5 foleys catheter.

Have taken ABG samples and other samples 

Have seen ICD insertion in a pateint

Learnt and did CPR for 3 pateints

Have done Ambu for 3 pts

Have learnt and did systemic examinations   Like CNS examinations in stroke pt.

Seen tracheostomy procedure for icu case

Thought my junior interns procedures like taking ABG and interpretation , foleys catheter insertion , ryles tube insertion , giving an IM injection , removal of foleys.

Guided junior interns in OPD auditing as said by HOD sir

Have done 10 PaJR groups and OPD auditing

Make a cumulative collaboration of OPD auditing as instructed by HOD sir
