June 5 , 2023 Week 2   DAY 1- Monday ( 5/6/23) [ 6/5, 10:04] Gauri 2k18 Kims: 20230604535 A 50 year old male came with C/o pain in the left side of chest since 2 days  Pain is of pricking type, radiating to left shoulders  Giddiness + Sweating + No palpitations and SOB H/o alcohol consumption and smoking. (Stopped 1 year back)  K/c/o HTN since 1 year. Patient is on medication  N/k/c/o DM ,Asthma, TB, Epilepsy , CVD O/E:  Pt is c/c/c  Afebrile  PR- 82bpm BP- 120/80mmHg   RR- 17cpm  CVS: s1,s2 heard ,no Murmurs,  RS:BAE+ ,no added sounds ,NVBS,   P/A: soft, non tender CNS:NFND   ECG normal  Treatment -  Tab. Pantop 40 mg OD x10days  Syp.
